Admission Requirements

Upon admission to the Daycare the following are required:

1. Completed application for daycare

2. Copy of child's immunization

3. Parent's signature on consent form for the examination and treatment of their child by a physician in the case of an emergency

4. Parent's signature to the agreement of financial policy terms.
